Care Management: Care Coordination Basic Skills Part 1

To succeed in today’s emerging healthcare models, such as health homes, patient-centered medical homes, and accountable care organizations – front-line healthcare staff members are increasingly being asked to serve as a bridge between patients and providers of healthcare and social services.

This course is part one of a two-part series that aims to provide an overview of care coordination, patient navigation, while also exploring the role healthcare front line staff have in delivering care coordination services.

Care Coordination is geared towards reducing the fragmentation of care provided while simultaneously supporting patients as they navigate the healthcare system ensuring that they receive the right care at the right time in the right place, which leads to improved patient outcomes and a better experience. Care coordination requires sharing information among interdisciplinary teams that include medical and social service providers who are all working together to improve the patients’ health and wellness. Finally, this course will provide healthcare front line staff with the information needed to implement care coordination and patient navigation skills in the day-to-day care of patients.
